Many are hoping for a solution that feels more natural and without side effects. My focus is to cut through the noise and give you a clear overview: what we know works, what’s questionable, what can be dangerous – and how to put together a realistic plan that actually helps you in everyday life.

“Natural medicine and prostate” typically refers to herbs, plant extracts and supplements that are used for:

  • Frequent urination, loose stream or night-time urination (often in benign prostatic hyperplasia – BPH)
  • Pelvic pain, heaviness, burning or discomfort during ejaculation (chronic pelvic pain/CPPS or chronic prostatitis)
  • Erectile dysfunction, reduced desire and performance concerns

I look at bladder emptying, residual urine and water habits – often simple changes (fluid timing, caffeine, “double urination”) make a noticeable difference along with targeted supplementation.
  • Prostatitis/CPPS: Inflammatory condition without bacteria in most cases. Often a combination of irritation, pelvic floor muscle tension and nerve sensitivity. Anti-inflammatory supplements can be relevant here, but the greatest effect typically comes when I work with muscles, nerves and habits – such as breathing, relaxation and graduated activity. This is where shockwave and EMTT treatment is a major contributor to a better quality of life and control.
  • Erectile dysfunction: Can be related to blood vessels, nerves, hormones – and to the mental effects of having persistent symptoms. Natural medicine has limited, but for some measurable support when properly selected and put into an overall plan. I always assess cardiovascular risk, sleep and stress response because it affects everything else.

What does the research say about natural medicine and the prostate?

I give you an honest overview here. Evidence varies – it’s important to know the quality and both potential and limitations.

Saw palmetto (saw palmetto/Serenoa repens)

  • Widely used for BPH symptoms. Some studies show mild to moderate symptom relief (less nighttime urination, better stream), especially with standardized extracts.
  • Other solid studies find no difference from placebo. The effect may depend on dose, extract quality and duration.
  • Safety: Often well tolerated. May slightly affect hormones. May cause stomach upset in rare cases. Choose standardized products.
  • Typical usage: 320 mg daily of an extract standardized to free fatty acids/sterols. I evaluate after 8-12 weeks via IPSS score and nighttime urination.

Beta-sitosterol

  • A plant substance from saw palmetto and other sources. Has relatively solid evidence of improving urinary symptoms in BPH.
  • Doesn’t necessarily shrink the prostate, but can improve flow and residual urine.
  • Safety: Generally good, but may interact with certain medications.
  • Practical: Doses around 60-130 mg beta-sitosterol daily are often seen in products.

Pygeum africanum (African cherry bark)

  • May reduce nighttime toileting and improve BPH symptom scores in some studies.
  • Safety: Mostly good, few gastrointestinal side effects reported.
  • Typical usage: 100-200 mg standardized extract daily.

Nettle root (Urtica dioica)

  • Used for BPH symptoms. Evidence is mixed, but some experience mild symptom relief, especially in combination with saw palmetto.
  • Safety: Generally good. May affect blood sugar and blood pressure in some.
  • Practical: 300-600 mg root extract daily.

Back pollen extract (Cernilton)

  • Interesting in chronic prostatitis/CPPS: Can reduce pain and pressure sensation in some men.
  • Safety: Typically good.

Quercetin and curcumin (turmeric) – anti-inflammatory support

  • In CPPS, studies show that quercetin can reduce pain and discomfort. Curcumin can support anti-inflammation and recovery.
  • The effect is often best as part of a combination strategy including neuromodulation, muscle relaxation and targeted training.
  • Practical advice: Quercetin 500-1,000 mg/d and curcumin 500-1,000 mg/d (often with piperine for absorption).

Green tea (EGCG) and lycopene (e.g. tomato)

  • Antioxidant effects. May have a modest positive effect on inflammatory markers and symptoms.
  • Not a standalone solution – more part of an anti-inflammatory dietary pattern.

Probiotics and the gut microbiome

  • The importance of gut bacteria for inflammation and pain is growing. Probiotics may be relevant for IBS-like symptoms and systemic inflammation, which can worsen pelvic pain.
  • The choice of strain and duration matters – I will decide based on your symptoms.

Natural remedies and erectile dysfunction: how realistic is it?

Natural medicine can stabilize the framework conditions: better sleep, less inflammation, lower stress response – all with a positive impact on erection. For vascular erectile dysfunction, however, this alone is rarely enough. Here I work purposefully with:

  • Focused shockwave therapy to stimulate new vascularization and tissue repair in the penis.
  • EMTT (electromagnetic transduction therapy) to affect tissue and pain with high-frequency magnetic energy.
  • Neuromodulation to reduce nerve pain and normalize pelvic floor muscle tone.

Natural remedies can support the process, but I will always be honest about what is likely to work for you – and in what order. Supplements I consider in selected cases include L-citrulline/arginine (nitric oxide support), Korean red ginseng and pinus bark extract. I only recommend them as supplements, monitor blood pressure and interactions and measure the effect on IIEF-EF and your perceived erection quality.

Safety, interactions and quality – how to avoid pitfalls

  • Quality and standardization: Choose products with proven content and dosage. Many “cheap” mixes contain too little active ingredient. I look for standardization, batch testing and clear declaration.
  • Interactions:
    • Blood thinners and blood pressure: Some herbs can enhance or counteract medication. If you have cardiovascular medication, I will review it before you start supplementation.
    • Hormone effects: Saw palmetto and nettle can affect hormone pathways. This is rarely a problem, but should be part of your plan.
  • PSA and red flags: Sudden worsening of symptoms, blood in urine/ semen, fever with chills, urinary retention or weight loss should be taken seriously. In these situations, I advise you to quickly contact your doctor for relevant tests.
  • Practical safety: Start low, change one thing at a time and keep a brief record of symptoms. Then I can quickly see what’s helping – and what’s not.

My approach in the clinic: natural medicine and prostate as part of a holistic plan

I start by listening. Your symptoms, your everyday life and your history set the direction. Then I use targeted diagnostics and a clear treatment plan.

Advanced diagnostics with ultrasound

I use ultrasound to assess tissue, blood flow and relevant structures in your area. This gives me a precise understanding of what I need to address: Is it primarily BPH symptoms, pelvic floor muscle pain, nerve irritation or a combination?

  • I often measure residual urine and look for signs of irritation or tight muscles.
  • For pain, I also test nerve sensitivity and tension patterns in the pelvic floor – gently and at your pace.
  • I couple the findings with simple scoring tools like IPSS (urination), NIH-CPSI (pelvic pain) and IIEF-EF (erection) so we can follow the development objectively.

Combination treatments: focused shockwave, EMTT and neuromodulation

  • Focused shockwave: Documented for erectile dysfunction and Peyronies, but also useful for stimulating healing and pain in general.
  • EMTT: Complements shockwave by magnetically affecting tissue and pain pathways.

When do I recommend natural remedies – and when not?

Frequently asked questions about natural medicine and prostate

How long should I try a supplement before I know if it works?

As a rule of thumb, 6-12 weeks with a standardized product and correct dosage. I monitor your symptoms via a simple score (e.g. IPSS for urinary symptoms, NIH-CPSI for pelvic pain) and objectively assess whether there is progress. No effect after that period? Then I adjust or stop – and don’t try three things at once.

Can natural medicine shrink an enlarged prostate?

In general, no. The best data primarily shows symptom relief (better flow, fewer nighttime toilet visits). If the goal is real tissue impact, it typically needs to be done via medical medication, lifestyle, weight loss and – for the erection part – vascular stimulating treatments like focused shockwave. My role is to choose the shortest, safest route to the desired effect.

Do natural remedies help with prostate cancer?

Natural medicine cannot replace oncology assessment or treatment. Some substances (e.g. lycopene or green tea) are being investigated for potential protective effects, but this is not treatment. If you have worrying symptoms or an elevated PSA, I advise you to talk to your GP about further investigation.

What about zinc, vitamin D and magnesium?

  • Vitamin D: Relevant in deficiency; may support immune regulation and muscle function.
  • Zinc: Important for hormone balance, but too high doses can cause side effects and affect copper status.
