Treatment of phimosis without surgery

Most men with phimosis first try to solve the problem themselves. Often with stretching, creams or products bought online. Some are quickly told that surgery is the only solution.

But the reality is more nuanced.

In many cases, phimosis can phimosis can be effectively treated without surgery – if the treatment is chosen correctly from the start. The problem is not a lack of solutions. The problem is that they are being used incorrectly or at the wrong time.

At MS Insight, the focus is not just on treatment, but on finding the solution that actually works in your situation – and avoiding unnecessary interventions.

Facts about phimosis:

  • Symptoms: Painful erection, difficulty urinating, redness/swelling, infection under the foreskin.
  • Causes: Can be congenital, due to skin condition or due to scar tissue after inflammation or wounds.
  • Treatment: Focused shockwave and Novoglan in combination for best effect.
  • Price: DKK 10,899 for a complete treatment course with Novoglan equipment and shockwave treatment over 8 weeks. If you only want the equipment, it costs DKK 2,499

Avoid circumcision: Effective treatment forphimosis

Phimosis occurs when the foreskin can no longer move freely over the glans. For some, it’s a mild tightness. For others, it affects function, comfort and sex life.

It can manifest as pain on erection, small tears in the tissue, reduced mobility or a feeling that something “won’t give”.

The cause is rarely one thing alone. Often it’s a combination of things:

  • Reduced elasticity in the tissue
  • Irritation or previous inflammation
  • Incipient scar tissue formation

Phimosis treatment – what really works?

When it comes to treatment, there are three main paths. But they are not equally relevant for everyone.

The non-surgical approach is the obvious first choice in many cases. This involves gradual tissue expansion, with the foreskin becoming more flexible over time. One of the most commonly used tools is Novoglanwhich is designed for controlled and gentle expansion.

But it’s important to understand that the result does not depend on the product alone. The effect depends on how the tissue responds – and how the treatment is adapted along the way.

Medical treatment with steroid cream can temporarily increase elasticity in some cases, but rarely stands alone as a solution.

Surgery may be necessary for severe narrowing or pronounced scar tissue. The challenge is that many get there without first trying a structured, conservative approach.

Why some phimosis programs succeed – and others don’t

The biggest difference between success and failure is rarely in the method. It lies in the assessment before treatment starts.

When treatment gets it wrong, it’s typically due to:

  • That the degree of narrowing is underestimated
  • That scar tissue is not identified
  • Tissue not responding as expected
  • Or that the progression is wrong

That’s why an MS Insight program always starts with a specific assessment of the tissue, elasticity and your situation.

Novoglan is used as part of a strategy

Novoglan is an effective tool for gradual foreskin expansion when used correctly.

A typical course involves daily treatment for around 30 minutes over 6-8 weeks. Clinical data shows high success rates and satisfaction among users.

But it’s not the device itself that matters.

What matters is that the treatment:

  • Starting at the right time
  • Adapts to the tissue continuously
  • Combine with what boosts response

When we combine treatment – here’s where the difference happens

In some cases, the tissue is too tight, too stiff or too affected to respond optimally to stretching alone.

Here I work on combining the treatment with shockwave and EMTT, which can improve tissue response and create better conditions for effect.

Shockwave contributes to increased blood flow and stimulation of local growth factors, while EMTT affects tissue metabolic activity and can improve elasticity.

In practice, this means that:

  • Tissue becomes more receptive to expansion
  • Treatment can be perceived as more effective
  • Progression can become more stable

It’s not necessary in all cases – but in the right situations, it can be the difference between limited and optimal effect.

Who is Novoglan treatment relevant for?

Non-surgical treatment is often a good option for men who experience initial or moderate phimosis and want to preserve the foreskin.

It is particularly suitable when the tissue still has some flexibility and there is no pronounced scar tissue formation.

What can you expect from a course?

When chosen correctly, you will typically experience a gradual improvement in tissue mobility and elasticity.

For many, this means:

  • Less discomfort
  • Better function during sex
  • Increased mobility
  • Full retraction option

The timeframe is often 6-8 weeks, but depends on the starting point.

Clarify your situation – before you choose a solution

If you experience tight foreskin, the most important thing is not to find a product.

It’s finding the right strategy.

At MS Insight you get a concrete assessment of:

  • Whether you can be treated without surgery
  • Which method is relevant
  • How to achieve the best possible effect

Total treatment pathway for phimosis

The clinic’s treatment course extends over 8 weeks from the first consultation, where, in addition to the examination, you will receive the first shockwave treatment and be instructed in the use of the Novoglan equipment.

You will use the Novoglan equipment for 30 minutes a day throughout the program. During the 8-week program, you will receive 3 additional shockwave treatments at 14-day intervals, where we will discuss the progress of the treatment.

The total treatment course, including the Novoglan equipment that you keep afterwards, costs a total of DKK 10,899.

Frequently asked questions about treating tight foreskin

It isn’t. Unfortunately, many men are told by their doctor that circumcision is inevitable when steroid creams fail. But surgery permanently removes up to 20,000 nerve endings, drastically changing sensitivity. Our combination treatment is created precisely to avoid the knife, so you can preserve your anatomy and full sensitivity.

When the foreskin becomes tight and inelastic, it is often due to microtrauma and the formation of microscopic scar tissue (fibrosis). The focused shockwave treatment goes in and “breaks down” this stiff tissue and significantly increases blood flow. Once the tissue is softened by shockwave, we use the Novoglan principle to provide a gentle, even and medically safe stretch. Shockwave makes the tissue cooperative, so the stretch works.

It’s very natural to be nervous about treatment in such a sensitive area, but the answer is no. The treatment is non-invasive (no needles or knives) and the intensity is always adapted to your pain level. The vast majority of men experience it as a slight tingling or prickling of the skin as the sound waves work.

The treatment is aimed at adult men who experience discomforts such as painful erections, difficult conditions for sexual intercourse or hygiene challenges due to a constricted foreskin. We do not treat children. During the initial (and non-binding) consultation, we will clarify your specific degree of phimosis to ensure the method is right for you.

Clinical studies* have shown that the Novoglan® Foreskin Tissue Expander medical device is effective in treating all degrees of phimosis in selected adult patients.

* Clinical data from Novoglan PMS 2021, a randomized sample of 811 patients. See the peer-reviewed scientific article published in the journal Translational Andrology and Urology:
https://tau.amegroups.org/article/view/115945/html

Lasting results can be achieved by applying a gentle and controlled stretch between the foreskin and glans (head of the penis) using Novoglan® medical devices daily for up to 30 minutes,

Although you may experience early improvements within a few weeks, treatment should be continued for at least 6-8 weeks for best results.

The effectiveness of Novoglan® Foreskin Tissue Expander for the treatment of phimosis depends on several factors, including the severity of the condition and the individual’s natural response to treatment.

In clinical studies, 90% of participants were able to fully retract the foreskin after 6 weeks of daily treatment.

It is important to follow the instructions carefully and be consistent with the treatment, as this increases the likelihood of the best possible result and a lasting effect.
