Here I go through clearly and concretely what to expect, what’s normal, when to be aware, and what safe alternatives are available if the medication isn’t right for you. Viagra is the trade name for the drug sildenafil. It belongs to the group of PDE5 inhibitors, which relax the blood vessels in the penis so that blood flows in more easily when you are sexually stimulated. This means that Viagra doesn’t create an erection by itself – it still needs desire and stimulation. The effect typically starts after 30-60 minutes and can last 4-6 hours. The full effect may depend on the dose, whether you’ve eaten (especially heavy/fatty foods), how much alcohol you’ve drunk and your general health.
I help you understand whether your erectile dysfunction is primarily vascular (blood supply), neurogenic (nerves), muscular (pelvic floor), hormonal or psychosexual – making the choice of treatment more accurate so you don’t take unnecessary medication or live with avoidable side effects.
Standard sildenafil doses are typically 25, 50 or 100 mg. I recommend starting low and adjusting if the effect is too weak or the side effects too pronounced – always a maximum of one dose per day. The tablet can usually be divided and taking it on a relatively empty stomach gives the most predictable effect. Some people benefit from trying the medication 2-3 times under different conditions (food, time, calm/stress) before the real effect can be assessed.
Most side effects of Viagra are mild and transient. Many pass after the first few times when the body has “gotten to know” the medicine. Generally, it’s in the first 14 days that you’ll notice the most side effects. The most common are:
Headache is the most common side effect. It is caused by the dilation of blood vessels, also outside the penis. Some people also experience heat or redness in the face and chest. Often it helps to drink water, take mild painkillers (if you can) or reduce the dose.
I also find that a calmer start-up without time pressure reduces the headache. Avoid sauna/hot baths close to intake, and try to keep caffeine and alcohol down in the same evening. If this doesn’t help, I can work with you to assess a switch to another medication or an alternative approach.
Nasal congestion and mild dizziness are related to mucous membrane and blood pressure effects. Some people experience palpitations without having heart disease – it’s uncomfortable but usually harmless. If you get really uncomfortable or already have heart symptoms, don’t ignore the signals.
Advice for these side effects: get up slowly, avoid large amounts of alcohol and skip very hot baths on the day you take sildenafil. If you experience repeated episodes, I’ll adjust the dose and timing or recommend alternatives to keep you safe.
A burning sensation behind the sternum or bloating is quite common. Try to avoid large, fatty meals close to consumption and avoid lying down immediately afterwards. A lower dose may also help.
Antacids can delay the absorption of sildenafil and thus delay the effect. If you’re using antacids, keep a few hours away. Small, light meals and a glass of water upon ingestion are usually best tolerated.
Back discomfort and muscle pain do occur, but are more common with tadalafil than with sildenafil. If you are particularly sensitive to this type of discomfort, I can talk to you about timing, alternative preparations or non-medicinal options.
Gentle stretching, warmth and light movement often provide relief. For some, it’s enough to lower the dose or move the time of ingestion so that muscle soreness doesn’t affect your sleep.
Some side effects require you to stop and seek emergency help. They are rare, but it is important that you are aware of them:
A painful, persistent erection over 4 hours is an emergency condition. Seek help right away. The sooner you get going, the better the tissues in the penis are protected.
Don’t try to “wait it out”. If you experience significant pain or a hard, persistent erection, call 112 or the emergency room – without hesitation.
Some people experience temporary color distortion (blue/green glow) or blurred vision – this usually stops once the medication is out of the body. Sudden vision or hearing loss, on the other hand, is rare but serious. If you experience it, seek immediate emergency assessment.
If you have known eye disease or a history of unexplained vision loss, I will discuss with you whether sildenafil is appropriate – or if another strategy is safer.
A significant drop in blood pressure can cause severe dizziness, cold sweats, nausea and fainting – especially if Viagra is combined with nitrates (“nitroglycerin”) or so-called “poppers”. Chest pain during or after sex is always a red flag: stop and seek emergency help.
I also recommend avoiding sildenafil in case of obvious infections with fever or severe dehydration, as it can increase the risk of unpleasant drops in blood pressure.
Swelling of the face/throat, difficulty breathing or severe hives require immediate treatment.
A significant part of “viagra side effects” is about what it is combined with and what health condition you have.
If you use nitrates (for chest pain) or take poppers (amyl/butyl nitrite), do not take Viagra. The combination can cause life-threatening drops in blood pressure.

Certain alpha blockers (for enlarged prostate/blood pressure) together with Viagra can increase the risk of dizziness. Often the problem can be solved with a lower starting dose and clear separation in timing – but this needs to be planned to keep you safe.
I typically recommend taking the alpha blocker at a fixed time and sildenafil at another time – e.g. morning/evening – according to an individual plan. If using drugs such as tamsulosin, doxazosin or terazosin, careful escalation is especially important.
Alcohol can both amplify the side effects and impair the erection itself. A single item is rarely a problem, but “party drugs” and high alcohol consumption are a bad cocktail. The same goes for stimulants and substances that affect the heart and blood pressure.
If you get red in the head, dizzy or have palpitations from alcohol easily, this is a signal to cut back on the days you use sildenafil.
Grapefruit can increase the amount of sildenafil in the blood and worsen side effects. This also applies to strong inhibitors of the CYP3A4 enzyme, such as certain antibiotics (erythromycin/clarithromycin), antifungals (ketoconazole/itraconazole) and some HIV medications (ritonavir). Here, dose and timing are particularly important.

Unstable heart disease, recent heart attack/stroke, very low blood pressure, severe liver or kidney disease and certain rare eye diseases may make Viagra unsuitable. If you have severe curvature of the penis (Peyronie’s disease) and a tendency to priapism, you should be aware. I will advise you on what matters you should discuss with your own doctor if I feel there is a need for examinations or blood tests that are not performed in the clinic.

For most healthy men, Viagra is safe when used correctly. The risk increases if you combine with nitrates/poppers, take too high a dose, or have unrecognized cardiovascular problems. That’s why it makes sense to look at the big picture: blood pressure, sleep, stress, fitness and your erection history before I make a plan with you.
As a rule of thumb: If you can handle moderate physical activity (e.g. walking at a brisk pace or climbing stairs) without discomfort, sex is typically safe – but if in doubt, I’ll assess the situation individually and refer you to your own doctor if necessary.
