Smoking and impotence: How cigarettes affect your erection

When men come to my clinic with erectile dysfunction, I often meet them with a great deal of frustration and uncertainty. I can understand that. A lack of potency hits us men hard on both self-image and masculinity. One of the factors I most often discuss with my clients is lifestyle – and tobacco is a big one. The link between smoking and impotence is one of the most well-documented causes of erectile dysfunction. However, it’s important for me to emphasize that I’m never here to point the finger or judge you. My job is to provide you with the necessary knowledge about what’s happening physiologically in your body and present you with the solutions that can help you get back to a well-functioning sex life.

Why does smoking lead to impotence?

To understand why cigarettes are so harmful to potency, we need to look at the mechanics behind an erection. An erection is essentially a matter of blood supply. When you’re sexually stimulated, the arteries in the penis need to expand quickly and efficiently so that blood can flow into the cavernous bodies and make the penis hard. At the same time, the veins need to close off so the blood stays in there until the act is over.

Smoking hinders this process on several fronts. The chemicals in tobacco smoke directly damage the cells lining the inside of your blood vessels (the endothelium). When these cells are damaged, blood vessels lose their elasticity and ability to expand when needed. As a result, the blood flow to the penis is significantly reduced and the erection is absent or too weak to complete intercourse.

Atherosclerosis in the small blood vessels

I clearly see how smoking leads to atherosclerosis throughout the body. Many people immediately think of the heart when they hear the word atherosclerosis, but the truth is that the penis often acts as the “canary in the coal mine”. The blood vessels in the penis are significantly smaller than those that supply the heart. Therefore, problems with blood flow will often show up as erectile dysfunction long before you notice symptoms from the cardiovascular system.

Unfortunately, when I scan my clients with ultrasound, I often see how calcifications and reduced elasticity in the vessel wall restrict blood flow. This means that even though the desire is there, the body physically cannot deliver the necessary pressure for a stable erection.

It’s not just about nicotine

Many people ask me if nicotine is the only culprit. Although nicotine causes blood vessels to contract – which in itself is problematic for an erection – it’s the cocktail of thousands of toxins in cigarette smoke that breaks down the vessel walls over time. This affects the production of nitric oxide, a substance that is essential for blood vessels to relax and fill with blood. Without enough nitric oxide, you won’t get a stable erection.

Smoking and impotence: Is the damage permanent?

The question I get most often when the conversation turns to smoking and impotence is: “Is it too late?” Fortunately, the answer is often no. The body has an amazing ability to repair itself if given enough time.

When you stop smoking, your body almost immediately starts to clean up. Blood pressure normalizes and the amount of oxygen in the blood increases. Over time, the function of the inner vessel walls will improve, making it easier for blood to flow to the penis. Studies show that many men experience a noticeable improvement in the quality of their erections when they give up cigarettes.

That said, years of smoking may have left its mark in the form of more permanent tissue damage or calcification that the body cannot completely remove itself. This is where I as a therapist can step in and support the process with modern medical technology.

Diagnosis and treatment at MS Insight

Focused shockwave and EMTT: Rebuilding tissue

To treat erectile dysfunction caused by poor blood flow, I use focused shockwave therapy. Here I use focused sound waves that are sent precisely into the tissue. These sound waves stimulate the formation of new small blood vessels (angiogenesis) and activate stem cells, significantly improving blood supply over time.

I often combine this treatment with EMTT (Electromagnetic Transduction Therapy). Where the focused sound waves work mechanically with the tissue, EMTT works on a cellular level by increasing the energy level of the cells and promoting healing and regeneration. This combination is particularly effective for men who have had a smoking lifestyle, as I actively help the body repair the damage that has been done to the blood vessels.

Frequently asked questions about smoking and impotence

Does this also apply to e-cigarettes and snus?

Unfortunately, yes, it does. Although e-cigarettes and snus do not contain tar, they still contain large amounts of nicotine. Nicotine is a vasoconstrictor, which means it causes blood vessels to constrict. This immediately reduces the blood flow to the penis, making it harder to achieve and maintain an erection. Therefore, nicotine products alone can also contribute significantly to impotence.

How quickly can I feel the effects of quitting smoking?

Improvements in blood circulation can often be measured within weeks of quitting smoking. Many men find that their morning erection returns or becomes stronger within the first few months. However, a full recovery depends on how long you’ve been smoking and the extent of the damage to the vessel walls.

Can shockwave remove the damage from smoking?

Focused sound waves (shockwave) are highly effective at stimulating the formation of new blood vessels and improving the function of existing ones. This means that the treatment can compensate for much of the damage caused by smoking by restoring blood flow to the penis. However, the best results are always achieved if the treatment is combined with smoking cessation, so we don’t “build with one hand and break with the other”.

Am I at risk if I only party smoke?

Even occasional smoking acutely affects your blood vessels. Every time you ingest nicotine and smoke, you stress the vascular system. Although the risk is lower than with daily smoking, binge smoking – especially in combination with alcohol – can lead to acute erectile dysfunction on the night and contribute to long-term vascular problems if done regularly.
