If you’re googling “Sildenafil vs. Viagra”, you’re probably in a situation where your erection isn’t working the way you want – maybe only sometimes, maybe not at all. Maybe you also have pelvic pain, abdominal tension or concerns about performance in bed. And you want to know if a pill can solve the problem – and if Sildenafil and Viagra are really just the same thing.

I work with men every day who struggle with erectile dysfunction, pain and uncertainty about their sexual function. In this article, I go through the difference between Sildenafil and Viagra, what you can realistically expect from the medication – and when it makes sense to look deeper into the cause and get a more targeted treatment that takes into account your body, nervous system and emotions so you don’t have to use the pills.

Sildenafil vs. Viagra – in a nutshell

‘Sildenafil’ is the name of the active substance. “Viagra” is a brand name. It’s actually the same type of medicine, so when you compare Sildenafil vs. Viagra, it’s more about form than content.

Viagra is the original branded product that was first on the market. Sildenafil is the generic version – a ‘copy’ medicine with the same active ingredient once the patent on the original has expired. It’s a bit like the difference between Panodil and “paracetamol from another brand”.

The important thing is: If you buy Sildenafil from an authorized pharmacy, the effect is basically the same as Viagra when the dose is the same. The difference typically lies in:

  • Price
  • Appearance (color, shape)
  • Excipients (the inactive substances in the tablet)
  • Brand and marketing

Some men feel more comfortable taking the ‘household name’, others are perfectly happy with generic Sildenafil. The most important thing to me is that you make an informed choice and use the medication safely and realistically – not what it says on the package.

How do Sildenafil and Viagra actually work?

Sildenafil (and therefore Viagra) belongs to a group of drugs called PDE5 inhibitors. They work by:

  • Relax the blood vessels in the penis
  • Increase blood flow when you are sexually stimulated (but there should still be a possibility of flow)

What it means:

  • The medicine doesn’t create desire – you still need to be turned on.
  • It does not automatically give an erection – there still needs to be sexual stimulation.
  • It can help the penis respond better to stimulation.

Many people are disappointed because they think the pill itself will “solve everything”. But if the cause is, for example:

  • Massive tension in the pelvic floor
  • Prolonged pelvic or penile pain
  • Reduced blood supply and calcifications.
  • Severe performance anxiety or stress
  • Significant nerve impingement after surgery or injury

… then Sildenafil/Viagra will often only help partially – or not at all. And then you can easily end up getting even more frustrated and worried because you might think: “If even Viagra doesn’t work, there’s probably nothing we can do.” In these situations, it’s even more important to find out what’s really going on and then treat the cause instead of the symptom.

Are Sildenafil and Viagra equally good?

When I look at “Sildenafil vs. Viagra” in terms of efficacy, the answer in most cases is: Yes, they are equally good – if:

  • The dose is the same
  • The medicine comes from an approved pharmacy
  • You are using it correctly

In rare cases, some men experience a difference, for example:

  • That they think the original (Viagra) works a little better
  • That they tolerate one tablet better than the other (in terms of side effects)

This can be due to differences in excipients, absorption or just expectations and comfort with a particular product. The mind matters more than many people think – if you feel more at ease with “the familiar”, that in itself can positively affect the experience.

In practice, many people start with Sildenafil (the generic version) because it is cheaper – and for the vast majority it works fine. If you are not experiencing a satisfactory effect, it makes sense to talk to your doctor about dose, timing and possibly try another drug in the same group or a completely different treatment strategy.

Standard doses for erectile dysfunction:

  • 25 mg – low dose

Whether it says “Viagra 50 mg” or “Sildenafil 50 mg”, it’s the same amount of active ingredient. Some people benefit from a lower dose, while others need to reach 100 mg to notice a clear difference. It should always be assessed individually based on your age, health and any other medications you may be taking.

How fast does Sildenafil/Viagra work – and for how long?

Most people feel the effect after:

  • Approximately 30-60 minutes – hence the advice: Take the tablet 1 hour before sex.

Duration of effect:

  • Typically 4-6 hours where you find it easier to get and keep an erection with sexual stimulation.

It doesn’t mean you have an erection all the time – it means that the “system” is more cooperative during that time if you are sexually stimulated. Fatty and heavy food in close proximity, alcohol or a lot of fatigue can delay or weaken the effect, and this is something I often talk to men about when Sildenafil or Viagra doesn’t immediately work as they had hoped.

Pros and cons of Sildenafil and Viagra

Possible benefits

  • Can improve erection relatively quickly
  • Can provide a sense of control and confidence in the situation
  • Can break a vicious cycle of performance anxiety in some men
  • Cheaper in generic form (Sildenafil) than original (Viagra)

For some, Sildenafil or Viagra becomes an important tool to rebuild confidence in the body and in sex – especially if the problems started after a bad experience, a breakup or a period of high stress.

Typical drawbacks and limitations

  • Doesn’t work if there is no desire or stimulation
  • Does not address underlying circulatory, nervous, pelvic floor or mental health issues
  • May cause side effects (e.g. headache, redness, heat sensation, acid-related discomfort)
  • Must be used in a planned way – some find it “unsexy” or inhibiting
  • Can become a “crutch” that you become mentally dependent on

I also find that some men start pushing themselves extra hard when taking medication – because they feel they “have to perform now that the pill is taken”. This can increase the pressure and actually make it harder to feel desire and pleasure. That’s why it’s important to look at both the medication and the overall situation around your sex life.

1. Is Sildenafil cheaper than Viagra?

Yes, you can. Generic products like Sildenafil are typically significantly cheaper than the original Viagra. Many people therefore choose Sildenafil for financial reasons – especially if they use it regularly.

When I talk to men about Sildenafil vs. Viagra, it’s often about finding a solution that is both practical in everyday life and sustainable in the long term – also financially. It has to be something you can stand behind and feel comfortable with.

2. Is it dangerous to buy Sildenafil or Viagra online?

There’s a big difference between buying:

  • Via approved Danish pharmacies
  • Via foreign websites without control

The latter can be downright risky. You’re taking a risk:

  • Wrong dose – too much or too little active ingredient
  • Contamination with other substances
  • No real effect – only “placebo pills”

I always recommend that you only use medication that comes through legal channels – and that you get assessed if it’s safe for you based on your health. It’s not just about treating erectile dysfunction, but about your heart, blood pressure and overall health.

3. Can I take Sildenafil/Viagra if I have heart problems?

Here you need to be extra careful. PDE5 inhibitors (Sildenafil/Viagra) must not be combined with nitrate preparations (e.g. nitroglycerin for angina pectoris) as this can cause a dangerous drop in blood pressure.

Do you have:

  • Known heart disease?
  • Chest pain (angina)?
  • Very low blood pressure?

  • That dose/timing/use is not optimal
  • That there are multiple factors at play simultaneously (physical + mental)

Here I take a systematic approach and assess:

  • Blood supply to the penis (incl. ultrasound scan)
  • Pelvic floor tension level and function
  • Nerve impingement (e.g. pudendal nerve)
  • Hormonal conditions, lifestyle, sleep and stress
  • Psychosexual factors, performance pressure, past experiences

Not physically addicted in the same way as with drugs. But:

This can keep you stuck in the “I can’t help myself” mindset, even though your body is actually capable of functioning better than you think. That’s why I often work with using medication as a temporary aid, while treating the causes and helping you regain confidence in your own body – step by step.

What if your problems are about more than erection?

  • Pain in the pelvis or around the penis
  • Burning, chasing or tightening sensations in the crotch
  • Problems emptying the bladder completely or frequent urination

  • Prolonged stress and nervous system overload

Here I work with a combination of:

  • Detailed examination including ultrasound scan
  • Focused shockwave therapy that creates new blood vessels and reduces the need for medication
  • EMTT (electromagnetic transduction therapy)
  • Neuromodulation – targeting nerves to relieve pain and tension
  • Pelvic floor training and relaxation
  • Advice on lifestyle, sleep, stress and sexual communication

The goal is for your body to be able to cooperate again – and for medication, if still relevant, to become a conscious choice, not an emergency solution. For me, it’s not just about you being able to have sex, but being present, feeling safe and having a sex life that makes sense to you.

When does it make sense to use Sildenafil or Viagra – and when not?

It can make sense if:

  • You have erectile dysfunction without severe pain or other complex symptoms
  • Your body is otherwise reasonably healthy and you have been thoroughly assessed for risk factors
  • You want a relatively simple, symptom-relieving solution to get going again
  • You are prepared to manage any side effects and not overdose

In these situations, Sildenafil or Viagra can be a great part of an overall plan where the goal is to give you a break, while you may also be working on exercise, weight, sleep, alcohol, smoking and other factors that also affect erection.

It’s often not enough if:

  • You have persistent pelvic pain, pain during sex or ejaculation
  • Your blood flow is reduced and there are calcifications in the penis
  • You have severe diabetes or neuropathy
  • You have pronounced nerve symptoms in the crotch or abdomen
