In short, phimosis is a condition where the opening in the foreskin is so narrow that it cannot be pulled back over the glans (head of the penis), or only with great difficulty and discomfort. For some men, the condition has always been there, but for many it occurs suddenly or gradually in adulthood due to tissue changes, skin conditions or scar tissue formation.
The problem is typically most apparent when the penis is erect. In a flaccid state, the skin may move fairly freely, but as blood flow increases and the penis grows in girth, the foreskin becomes a tight ring that squeezes. It’s like wearing an elastic band around your wrist that’s too small; it creates significant discomfort, inhibits blood flow and, at worst, can cause pain that kills any desire for intimacy. When the skin is not supple, it loses its function and this affects the whole experience of being a man.
The symptoms of phimosis vary from man to man, but the common denominator is that it interferes with natural function and pleasure. The most common signs I see and treat in the clinic include:

One particular and often overlooked cause is the skin condition Lichen Sclerosus. It is an autoimmune condition that manifests itself as a whitish, thin, yet hard or “papery” ring on the foreskin. Although I’m not a dermatologist, I have a lot of experience in recognizing the tissue changes and my treatment can often significantly relieve symptoms by softening the tissue and increasing blood flow, making the skin more resilient again.
Many men fear that the only answer to phimosis is circumcision. While surgery may be necessary in very severe cases, it is far from always the first choice.
