In my clinic, I meet many men who have lived with this problem for years, often because the thought of surgery seems scary or because it feels embarrassing to talk about. Novoglan is a medical device designed to treat phimosis gently and without surgery. As a practitioner, I’m a big believer in always exploring non-surgical avenues first. The foreskin has an important function for sensitivity, and if we can solve the problem without cutting, it’s always preferable. In this article, I will explain what exactly Novoglan is and how I can help you assess whether it is the right path for you – possibly as a complement to my other treatments in the clinic.

Novoglan is a system designed to treat tight foreskin through controlled tissue expansion. The principle is simple but biologically ingenious: When we subject skin tissue to a gentle and sustained stretch, the cells respond by dividing and creating new tissue. Over time, this makes the foreskin more spacious and elastic.
The device itself consists of a small, specially designed balloon that is inserted under the foreskin. Using an attached pump, the balloon is filled with air until you feel a firm stretch. The key here is that it’s painless. You control the pressure 100% yourself. This minimizes the risk of tearing or injury, which unfortunately often happens when men try to stretch the foreskin manually with their fingers.
When I guide my clients in the use of this equipment, I always emphasize patience. We are working with a biological process. It’s not a quick-fix that works in two days, but with dedicated effort, many people can avoid circumcision.

The target group is broad, but in my view it is important to distinguish between the causes of the problem. I typically see two groups of men who benefit greatly from this method:
However, it’s crucial to find out why your skin is tight. If you suffer from the skin disease Lichen Sclerosus (an autoimmune condition that forms whitish, hard scar tissue), stretching alone is rarely enough. This requires a more specialized approach, where I often have to soften the tissue with the clinic’s technologies before stretching is even possible.
Why spend time on a device like Novoglan when you can “just” get circumcised? I get that question a lot. The answer is about quality of life and sensation. The foreskin is packed with thousands of fine nerve endings and acts as a protective sheath for the head of the penis (glans).
Circumcision permanently removes this sensitive tissue. For many men, this means a changed sexual experience, and the glans will over time become more “hardened” (keratinized) and less sensitive because it constantly rubs against fabrics. By taking a conservative approach where I help you expand the skin you have, we preserve the natural anatomy and thus the full potential for pleasure.

Although Novoglan is an excellent tool, it is not a miracle cure if the tissue quality is very poor. In my clinic, I often see men whose foreskin has become thickened, fibrous or scarred over many years. In these cases, the tissue is simply too stiff to respond to the balloon alone.

Before I make a plan, I always do a thorough examination, often supplemented by ultrasound scanning. I don’t guess; I want to see what’s going on. With ultrasound, I can measure the thickness of the foreskin, check for calcifications (plaque) or deep scar tissue, and assess blood flow.
This diagnostic precision is the foundation of my treatment. It gives you peace of mind. If I see signs of severe Lichen Sclerosus or other pathology on the scan that requires medical intervention, I will advise you exactly how to talk to your doctor about a prescription ointment to complement my treatment.
If we decide that Novoglan is the right tool for you, discipline is key. It’s not about “squeezing the lemon”. On the contrary, overuse can lead to microscopic tears that heal as tighter scar tissue – making the pain worse.

I regularly see men who come to me with erection problems where it turns out that the root of the problem is actually a tight foreskin. The body remembers pain. If it has hurt or tightened uncomfortably when you have an erection for a long time, your brain will subconsciously start to work against the erection to protect you.
That’s why at MS Insight I never just work with the symptom in isolation. I look at the whole picture. If you’ve been tensing up your pelvic floor for fear of pain, loosening the foreskin alone won’t help. We also need to relax the pelvic floor and restore trust in your body. Does it hurt to use Novoglan?
No, it should absolutely not hurt. If it does, you’ve inflated the balloon too hard. The treatment should feel like a firm stretch, but be painless. Pain is the body’s signal that you risk damaging the tissue.
How quickly do I see results?
It’s very individual and depends on how tight and thickened the skin is. Some notice a difference after a few weeks, while others need to use the system for a few months. Patience and continuity are key.
Can I use it if I have Lichen Sclerosus?
Yes, but with great caution. Tissue affected by Lichen is more fragile and vulnerable. Here I would strongly recommend that you combine it with shockwave therapy in the clinic to make the tissue supple enough to withstand the stretch.
Is the result permanent?
For the vast majority, once the tissue has expanded sufficiently, the result is permanent. However, scar tissue has a tendency to want to “contract” again, so I often recommend maintaining with a stretch once in a while afterwards.
