When I see men in the clinic who struggle with erectile dysfunction or unexplained pelvic pain, the conversation quickly turns to one thing: Blood flow. This is where we find one of the most important but often overlooked factors in your intimate health: Nitric oxide. It may sound technical, but the reality is that this little molecule is absolutely crucial to your ability to perform and to keep your abdomen pain-free.
I often find that my clients are in control of their testosterone, but most have no idea what nitric oxide does for them. At my clinic, MS Insight, I see it as my primary task to make you more knowledgeable about your own body. Nitric oxide is the signal substance that tells your blood vessels to relax and open up. Without sufficient nitric oxide, it’s physiologically impossible to achieve a hard erection, no matter how much desire you have in your head. Here I explain why nitric oxide is so important to you and how I work to optimize your levels so you can get your function back.
Nitric oxide (NO) is a gas that your body has to produce itself. This happens primarily in the innermost layer of your blood vessels, which we call the endothelium. Its main function is vasodilation – a technical term for the dilation of blood vessels. I like to compare it to a garden hose that has a kink; the water can’t flow through. Nitric oxide is the substance that straightens the hose and makes it wider so that blood can flow freely, powerfully and unhindered into the tissue.
When your body is working properly, nitric oxide is released the second you are sexually stimulated. This sends a lightning-fast signal to the smooth muscles of the penis to relax. Only when the muscles relax can the blood fill the cavernous bodies and the result is a strong erection.
But nitric oxide does more than that. It plays a huge role in your immune and nervous systems. For the conditions I treat – such as chronic pelvic pain and CPPS (Chronic Pelvic Pain Syndrome) – nitric oxide is crucial because it provides oxygen and nourishment to tissues and nerves. Without oxygen, the tissue cannot heal and this creates a breeding ground for pain.
Erectile dysfunction is very often the first and earliest sign that your nitric oxide production is impaired. Many of the men I see in the clinic have already tried potency pills like Viagra or Cialis without much success. It’s important to understand that these pills do *not* create an erection by themselves. They only work by amplifying the effect of the nitric oxide you *already* have in your body by preventing it from being broken down too quickly.
The problem occurs if the inside of your blood vessels (the endothelium) is so damaged or “sluggish” that it produces too little nitric oxide to begin with. Then the medicine simply has no material to work with. This is where I come in. My approach is not about treating symptoms with more pills, but about restoring the body’s own ability to produce nitric oxide and form new, healthy blood vessels.
If you find that your erection has become flaccid or that it disappears quickly during the act, it’s a classic sign of “endothelial dysfunction”. In short, this means that your blood vessels are not releasing enough nitric oxide. It’s a condition I take seriously, as it’s also indicative of your overall cardiovascular health.
Nitric oxide is not just about erectile function. I treat many men with debilitating, chronic pelvic floor pain or irritation around the pudendal nerve. In these cases, the tissue is often characterized by severe tension, poor blood circulation and a state of hypoxia.
When tissue lacks oxygen, it sends alarm signals in the form of pain. Nitric oxide is the key to opening up microcirculation – the tiny capillaries that supply nerves and muscles deep in the pelvis. By increasing nitric oxide levels locally, I can significantly improve blood flow, flush away inflammatory substances and reduce pain. This is an essential part of healing as we work to restore a healthy pelvis.
Unfortunately, it is a biological fact that our production of nitric oxide decreases with age. Already from the age of 40, many men only produce about half of the nitric oxide they did when they were 20. But age is not the only factor and lifestyle plays a huge role.
The typical causes I see that inhibit your NO production include:
Stress: High levels of the stress hormone cortisol are poison to your NO production.
Physical inactivity: Blood vessels need to be “exercised” and squeezed to stay elastic and healthy.
Diet: A diet poor in vegetables and rich in processed food doesn’t provide the body with the right building blocks.
Smoking: Smoking directly damages the cell layer (endothelium) to be affected by the drug.
Mouth breathing: A large part of our nitric oxide is actually produced in the nasal cavity. If you breathe through your mouth (especially when you sleep), you miss out on this vital production.
At MS Insight, I base my treatment on a combination of the latest technology and practical advice. My goal is to treat the cause of the problem, not just the symptom. We need to get the “engine” running again.
One of my absolute core competencies is the use of focused shockwave. I only use focused sound waves because they are the most potent and precise form of shockwave – as opposed to the radial type often used by regular physiotherapists. When these sound waves hit the tissue in the penis or pelvic floor, there is an immediate release of nitric oxide and a rebuilding of the vessel wall over time.
But even more important is the long-term effect: the sound waves stimulate the formation of stem cells and growth factors that cause the body to form new blood vessels (angiogenesis). This means that we physically renovate the infrastructure in your abdomen so that you can eventually produce and transport more nitric oxide when you need it.
I often supplement the treatment with EMTT (electromagnetic transduction therapy). This technology works at the cellular level by increasing the energy metabolism of the cells. It optimizes the environment around the blood vessels and nerves, making them better able to respond to the nitric oxide signals sent by the body.

Eat your nitrates:
Certain foods are rich in nitrate, which the body efficiently converts into nitric oxide. Beetroot (and especially concentrated beetroot juice), rocket, spinach and dark chocolate are excellent sources. I often see a measurable effect when my clients start taking this seriously in their everyday lives.
Breathe through your nose:
It sounds simple, but it’s extremely effective. The nose produces nitric oxide, which is carried down into the lungs and out into the system, oxygenating the blood better. Practice breathing through your nose both when you exercise and when you relax.
Exercise:
Regular exercise, especially the kind that gets your heart rate up, forces the endothelium to produce more NO to keep up with oxygen demand. This keeps your blood vessels supple and “awake”.
I get a lot of questions about supplements like L-arginine and L-citrulline. These are amino acids that act as ‘raw materials’ for the formation of nitric oxide. For some men they can have an effect, especially L-citrulline, which is well absorbed in the body.
However, my position is clear: supplements can rarely stand alone if you have real, physiological problems with erection or pain. If the “factory” (your blood vessels) is damaged, simply delivering more raw materials to the gate won’t help. We need to repair the factory first – for example with focused sound waves – so the system can function again. If you’re considering supplements, we can talk about it in the clinic and I can guide you on what specifically to ask your doctor to check in a blood test so we’re not shooting blind.
Unfortunately, erectile dysfunction or pain in the lower abdomen rarely goes away on its own. On the contrary. Lack of blood flow can, over time, cause the tissue in the penis to shrink or lose its elasticity (atrophy), making it much harder to treat the longer you wait.
Nitric oxide is the engine of your sexual health. If it’s stalling, it needs to be attended to before it stalls. I meet you at eye level and with me, no question is too embarrassing – I’ve heard it all before. I know it can be daunting to book that first appointment, but I also know how much relief and quality of life my clients experience when we find the solution.
Is it dangerous to increase your nitric oxide levels?
No, when we talk about natural stimulation through diet, lifestyle and shockwave therapy, it’s not dangerous. On the contrary, it’s healthy for your entire circulatory system and your heart. If you are taking certain heart medication, we will of course take this into account during your first consultation.
How quickly do the treatments work on my NO levels?
When treated with focused sound waves, nitric oxide is immediately released into the tissue. Many people notice a change shortly after treatment, but the big, lasting effect is built up over the following weeks and months as the body forms new blood vessels.
Can’t I just take Viagra to fix the NO problem?
Viagra (PDE5 inhibitors) do not create nitric oxide; they simply slow down its breakdown. If your body produces very little nitric oxide due to poor blood vessels, the pills will often have limited or no effect. My treatment aims to optimize the production itself, potentially making you independent of pills or making them work significantly better again.
Does beetroot juice really work?
Yes, it actually does. But you need very large amounts. Beetroot is very high in inorganic nitrate, which is converted to nitric oxide in the body. It can be a great addition to my treatment, but it rarely solves severe cases of erectile dysfunction on its own.
