Enlarged prostate and age: Should you accept it or act?

It’s one of those topics that men are often reluctant to talk about out loud. But the fact is that for many men, an enlarged prostate and age are inextricably linked. As a therapist at MS Insight, I meet men every day who have accepted frequent trips to the toilet, interrupted sleep and declining potency as an inevitable part of getting older. My message to you is clear: it doesn’t have to be.

Although it’s a natural physiological process that the prostate grows over the years, it doesn’t mean you have to live with a significantly reduced quality of life. In my clinic, I see it as my primary task to demystify the condition and offer you concrete, effective solutions. I want to help you get back to an everyday life where you don’t have to constantly plan your day around the nearest toilet or worry about your ability in the bedroom.

What happens to the prostate when age takes its toll?

The prostate, or bladder neck gland, is a small walnut-sized gland that sits just below the bladder and surrounds the urethra. Its primary function is to produce the fluid that transports sperm. When I talk to my clients about prostate enlargement and age, I most often refer to the condition *Benign Prostatic Hyperplasia* (BPH) – a benign enlargement of the gland.

It is crucial for me to emphasize the word “benign”. It has nothing to do with cancer. It’s a hormonal growth that starts in most men in their 30s and 40s, but symptoms typically don’t appear until later in life.

As the prostate grows, it can squeeze the urethra, just like stepping on a garden hose. This creates resistance when emptying the bladder. For many men, this becomes a slippery slope: the bladder muscle first thickens to compensate for the resistance, but over time it risks becoming flaccid and inefficient. This is where I come in – because I want to help you intervene before bladder function is permanently compromised.

Symptoms of an enlarged prostate you shouldn’t ignore

Many of the men I treat have been dealing with the symptoms for years. They’ve gotten used to it, or mistakenly believe that “that’s just being a man over 50”. Maybe you know it yourself? It starts insidiously. The beam gets a little looser. You have to push a little harder to get going.

The classic symptoms I see in the clinic include:

  • Difficulty starting: It takes time for urination to start, no matter how needy you are.
  • Slack beam: There is not the same pressure as before; the arch pressure is missing.
  • After-drip: It’s hard to empty your bladder completely and you experience annoying drops in your pants afterwards.
  • Frequent urination: Especially at night (nocturia), which affects your sleep quality, mood and energy levels the next day.
  • Sudden, strong urge to urinate: You have to run when the urge arises and can’t “hold it”.

Unfortunately, it doesn’t always stop there. In my practice, I see a clear link between an enlarged prostate and pelvic pain or erectile dysfunction. When the bladder and pelvic floor are constantly working overtime to pass urine, tension and inflammation occurs. This affects your entire lower abdomen and your quality of life.

Erectile dysfunction and enlarged prostate are linked

The link between *Lower Urinary Tract Symptoms* (LUTS) – urinary problems – and erectile dysfunction is too rarely discussed. But the link is well documented and I often see it in my clients.

When you struggle with an enlarged prostate, your pelvic floor is often tense and stressed. A healthy and hard erection requires a calm nervous system and a relaxed pelvic floor so that blood can flow freely into the enlarged organs. If the area around the prostate is characterized by inflammation or physical pressure, it negatively affects blood flow and nerve conduction to the penis.

Many men think that their erectile dysfunction is solely due to age, but it is often a “collateral damage” of untreated prostate problems. That’s why at MS Insight I work holistically. I can’t just treat the symptom (erectile dysfunction) without looking at the cause (prostate and pelvic floor condition).

Diagnostics: I give you certainty with ultrasound

When you come to my clinic, we don’t start by guessing. I place great emphasis on accurate diagnostics, which is why I use advanced ultrasound scanning to visualize the prostate, bladder and pelvic floor.

With a scan, I can accurately assess the size (volume) of the prostate, its shape and whether it bulges into the bladder (intravesical protrusion). I also check if you are emptying your bladder properly (residual urine), as standing urine in the bladder can be a breeding ground for infections and form bladder stones.

This visual walk-through is often an eye-opener for the men I help. Seeing your own anatomy on the screen makes it easier to understand why your body reacts the way it does. It creates confidence and removes a lot of the uncertainty that many men have.

Important: If during our conversation or examination I find signs that require further medical investigation – for example in relation to PSA (prostate-specific antigen) levels – I will always provide you with thorough guidance on how to talk to your own doctor. I do not perform cancer diagnostics, but I will help you navigate so that you get the right tests if deemed necessary.

Effective treatment without a knife: Shockwave and EMTT

Traditionally, the treatment for an enlarged prostate has been medication (which unfortunately can have side effects such as decreased sex drive) or surgery (where some of the prostate is “planed” away). But there are alternatives that regenerate rather than just alleviate symptoms.

In my clinic, I specialize in combination treatments that address the tissue, nerves and blood vessels without surgery.

Focused shockwave therapy

I use focused shockwave – and here it’s important to make a distinction. These are not the radial shockwaves you often see at the physiotherapist for a sore shoulder. I use focused sound waves that can penetrate deeply and precisely into the tissue around the prostate and pelvic floor.

These high-energy sound waves stimulate the formation of new blood vessels (angiogenesis), reducing inflammation and softening any scar tissue or calcifications in the area. In relation to prostate genes, the treatment can help reduce the inflammatory condition (prostatitis) that often accompanies enlargement, thus relieving both pain and difficulty urinating. At the same time, it has an extremely positive effect on erectile function as blood flow is optimized.

EMTT (Electromagnetic Transduction Therapy)

As a powerful supplement, I often use EMTT. This is a technology that works at the cellular level using powerful magnetic fields. Where shockwave is a mechanical effect, EMTT is an electrophysiological effect. It increases the energy level of the cells and effectively suppresses inflammation throughout the pelvic area. It is a completely painless treatment that you can receive fully clothed and is particularly good for chronic pelvic pain conditions (CPPS), which many men with prostate problems unfortunately suffer from.

Your lifestyle affects both enlarged prostate and your age

Even though I have access to advanced technology, I also need to look at the whole person. An enlarged prostate and age are one thing, but your lifestyle can either worsen or significantly alleviate symptoms.

1. Fluid intake: Many people drink too much coffee or alcohol, which irritates the bladder, or consume large amounts of fluids just before bedtime, which provokes nighttime urination.
2. Physical activity: Sedentary work is poison for the pelvic floor. We need to get blood circulation going to keep the tissue healthy.
3. Bowel function: A constipated bowel physically presses on the bladder and prostate, exacerbating voiding difficulties.
4. Cold: Many men experience acute exacerbation when they get cold around the pelvis or feet.
