An amazing experience at OCC ISMST in Bogotá – Shockwave, EMTT and the future of ED treatment

I’m enjoying a great time at the OCC ISMST in Bogotá with new and old friends. It has been an extraordinary experience to attend this congress where some of the leading experts in shockwave therapy (ESWT) and EMTT. I had the pleasure of being the keynote speaker in the urology section where I spoke about shockwave for erectile dysfunction and peyronies.

Thanks to Carlos Leal and an unforgettable week

A special thank you to Carlos Leal for your generosity and hospitality. It was an honor to be invited to your home, your clinic and this important congress. Having the opportunity to speak to more than 300 attendees about the latest research and practice in ESWT and EMTT for erectile dysfunction and Peyronie’s disease was an experience I will never forget.

Colombia was an amazing experience, not only professionally but also culturally. Experiencing Bogotá, meeting inspiring people and having deep conversations with some of the biggest names in the shockwave world, a company I am a part of, was a huge pleasure.

Why and how does ESWT work on erectile dysfunction?

ESWT (Low Intensity Extracorporeal Shockwave Therapy) has emerged in recent years as a groundbreaking and non-invasive treatment for ED. By stimulating the body’s natural healing processes, ESWT works on a cellular level to restore blood flow and improve erectile tissue function.

The mechanisms and effects of ESWT:

  • Mechanosensors are activated, stimulating regenerative response.
  • Neoangiogenesis – the formation of new blood vessels that improve blood flow to the penis.
  • Activation of progenitor cells, stimulating tissue healing and regeneration.
  • Improving microcirculation, which is essential for a stable and lasting erection.
  • Nerve regeneration, which is particularly important for patients with neurogenic ED problems.
  • Remodeling of erectile tissue, allowing the tissue to regain elasticity and function.
  • Reduction of inflammation and cellular stress responses, which is essential for long-lasting results.

These mechanisms make ESWT an effective and promising treatment for ED patients, especially those who experience inadequate response to PDE5 inhibitors (Viagra, Cialis, etc.).

Scientific evidence for ESWT’s effect on ED

Several studies confirm ESWT’s efficacy in ED, both alone and in combination with other treatments:

  • LI-ESWT is safe and effective for the treatment of ED in both PDE5I-responders and non-responders. (Vinarov, 2019)
  • LI-ESWT can improve sexual and erectile function in patients with ED after radical prostatectomy (RARP). (Hinati, 2023)
  • The IIEF-5 score and EHS improved in patients who underwent treatment and the results were maintained after 12 months of follow-up. (Cocci, 2023)
  • 57% of men with ED experienced an effect of a 5 treatment protocol with LI-ESWT. (Lund, 2013)
  • Improved energy output (EFD) can result in better treatment outcomes. (Porst, 2020)

However, research also shows that:

  • LI-ESWT can be an effective but often short-term solution for patients with cardiovascular risk factors or controlled hypertension. (Sikic, 2022)

Therefore, combining ESWT with other approaches, such as EMTT, hormonal interventions or lifestyle changes, is crucial to ensure optimal and long-lasting results.

Combining ESWT and EMTT – A new era in ED treatment

One of the most promising developments we discussed at the congress is the combination of ESWT and EMTT (Electromagnetic Transductive Therapy). While ESWT primarily works with mechanical stimulation of the tissue, EMTT has been shown to further enhance biological regeneration processes by:

  • Increase cellular energy production (ATP synthesis)
  • Reduce inflammation at deeper levels
  • Improve microcirculation in the tissue

This opens up new and improved treatment protocols that can potentially provide better and longer lasting outcomes for patients with ED and Peyronie’s disease.

What does all this mean for the future?

After a week of exciting discussions, presentations and scientific exchange, it is clear that shockwave therapy and EMTT will remain at the forefront of ED treatment. Recent advances show that we have only scratched the surface of what these technologies can achieve.

Personally, I am excited to continue my work to optimize treatment protocols and combine ESWT with other innovative methods to ensure our patients receive the best possible treatment.

Conclusion: ESWT and EMTT represent a revolution in the treatment of ED and Peyronie’s disease. With continued research and optimization of protocols, these technologies will play an even greater role in the future. The combination of mechanical stimulation, biological regeneration and personalized treatment plans will provide patients with much better and longer lasting results.
